Discharge Planning starts prior to admission, and our partnership with local hospitals ensure a smooth transition. Patients and caregivers participate in a discharge planning meeting within 72 hours of admission. Our team uses evidence-based clinical pathways to optimize outcomes to best manage the patient's quality of life.

Holy Family Manor is leading the way in achieving positive outcomes in sub-acute rehabilitation. Our sub-acute care/short-term rehabilitation unit is staffed by board certified geriatricians from St. Luke's Senior Care Associates and Lehigh Vally Physicians Group Geriatric Medicine.

Respiratory Health Services

Respiratory Health Services provides care to those patients with complex pulmonary care requirements, tracheostomy and other pulmonary care needs. Respiratory Therapists are on-site 8 hours per day/7 days a week.

Introduction of Respiratory Therapy - the scope of practice...

  • Assess lung and breathing disorders, recommending treatment methods in conjunction with attending physician.
  • Educating patients and families about lung disease to maximize their recovery.

Outcomes of Respiratory Therapy:

  • Decrease in unplanned re-hospitalizations
  • Increased tolerance to perform longer durations of therapy directly related to improved respiratory function
  • Increase cardiopulmonary endurance
