Social Services

The social services department advocates for each resident in areas of residents' rights and quality of life issues. They are a liaison between the resident, their families, and the facility staff to ensure a maximum level of comfort in each resident's day-to-day life

Assistance with mail, telephone hookups and application for financial assistance are offered, as well as referrals to supplemental support services including, but not limited to, Psychiatry, Psychology, Blind Association, Hearing Impairment, Hospice Services and Transportation Needs.

The social service staff promotes short-term residents to set goals that will enable a safe and supportive discharge and will coordinate all agreed upon services prior to their advancement to a lesser-care setting.

An important service is educating residents and their responsible parties about available community services such as Meals-On-Wheels, Homecare, Emergency Response Systems, Residential Services, Adult Day Care Services, Senior Centers, Private Duty Personal Assistance, Waiver Programs, and more.

Social services will also assist in ordering any necessary durable medical equipment prior to discharge as wheelchairs, walkers, commodes, transfer bags, or other equipment as needed.
