The social services department advocate for each resident in areas of residents' rights and quality of life issues. They are a liaison
between the resident, their families, and the facility staff to ensure a maximum level of comfort in each resident's day-to-day life.
The social workers welcome each resident and their family the day of admission to offer reassurance, emotional support and various levels of
education about Holy Family Manor and the services to which they are entitled. They monitor adjustment to temporary or long-term placement as well as to their changing health status.
Assistance with mail, telephone hookups and application for financial assistance are offered, as well as referrals to supplemental support
services including, but not limited to, Psychiatry, Psychology, Blind Association, Hearing Impairment, Dental Services and Transportation Needs.
Social workers promote short-term residents to set goals that will enable a safe and supportive discharge and will coordinate all
agreed upon services prior to their advancement to a lesser-care setting.
An important service is educating residents and their responsible parties about available community services such as Meals-On-Wheels,
Homecare, Emergency Response Systems, Assisted Living, Adult Day Care Services and Senior Centers.
